Histiocytic Necrotizing Lymphadenitis of the Left Parotid Area





Abstract
Histiocytic necrotizing lymphadenitis is relatively uncommen in the parotid area. The careful histopathologic examination should be exerted in order to establish the diagnosis of histiocytic necrotizing Iymphadenitis, because of its histopathologic similarity to malignant Ifmphoma.
A 25-year-old frmale visited the Department of Oral and maxillofacial surgery, college of dentistry, Seoul National University, complaining of bean nut size painless swelling on the left parotid area. The swelling was noticed 3 years ago, and had not increased in size, around admission. Punch biopsy on the parotid swelling showed sheets of macrophages laden with necrotic debris on the background of patch distibution of immunoblasts and pIasmacytoid Iymphocytes confirmed as T cell origin by positive immunoreactivity to Uchl-1. These findings are highly typical of histiocytic necrotizing Iymphadenits. MRI showed bilateral swelling of jugular digastric Iymph nodes.
